Signing this application creates a contract between MoPRIDE, Inc. and you, and you agree that you have read and abide by the information, Guidelines, Rules, and Code of Conduct for all participants, helping or working with you. FORCE MAJEURE: MoPRIDE, Inc. shall not be liable for any delay or its performance of its obligations and responsibilities under this agreement due to causes beyond its control, including, without limitation, weather, interruption or failure of telecommunications, or digital transition links, war strikes, lockouts or work stoppages or other labor difficulties, energy failures, embargo, national, regional or local emergency, insurrection or riot, acts of public enemy, fire, flood, or other natural disaster. Further, in the event that MoPRIDE, Inc. is unable to meet its obligations hereunder because of such Force Majeure Event, and the World AIDS Day is canceled, MoPRIDE, Inc. shall have no obligation to reschedule it or to refund any fees paid to it hereunder.
Vendors must supply their own tables and chairs
Vendors are responsible for collecting/reporting all sales and income taxes for yourself and/or your business
All participants will conduct themselves in a well-behaved and lawful manner always
Participants are responsible for all members of their party
The set-up period starts at 11:00 a.m., Sunday, December 1st, 2019
All booths must be set up by 12:30 p.m., Sunday, December 1st, 2019
Booths must stay open until 7:00 p.m., Sunday, December 1st, 2019
Dress Code: Clothing is required at this event; nudity in part or in whole is not permitted.
No firearms or weapons are allowed
No illegal drugs or substances allowed
Vendors are responsible to keep their booth area clean and trash put in proper receptacles
Children under the age of 12 must be accompanied by an adult
No soliciting without permission of the festival administration
All Vendors are responsible to have proof of appropriate licenses, permits, inspections for this event
No sales of bottled water, soda, alcohol or commercially-available beverages are permitted
Vendors are prohibited from having loud amplified music or other sounds, including a voice from their assigned space as a courtesy to fellow and surrounding vendors unless you have prior approval.
Each participating vendor must provide MoPRIDE, Inc. with an appropriate insurance certificate.
Certificate of Insurance
Vendors must provide general liability insurance with a minimum limit of liability per occurrence of $1,000,000 for bodily injury and property damage. Vendors must list the following information on the Certificate of Insurance in the area titled “Description of Operations” and Endorsements: MoPRIDE, Inc., its Officers, Agents, Employees & Volunteers are named as additional insured per the attached endorsement. Project: World AIDS Day 2019. Vendors shall provide MoPRIDE, Inc. with separate endorsements evidencing proof of MoPRIDE, Inc.’s additional insured status as to both the general liability and automobile liability insurance policies. If applicable, Automobile Liability insurance with a minimum limit of liability per occurrence of $1,000,000 for bodily injury and property damage. This insurance shall cover any automobile for bodily injury and property damage. If applicable, Proof of Worker’s Compensation Insurance for vendors who will have employees working during the event. All endorsements referenced above must include the applicable policy number.
I agree to hold MoPRIDE, Inc., its agents, officers, employees, and volunteers, harmless from and save, defend and indemnify them against any and all claims, losses, liabilities and damages from every cause, including but not limited to injury to person or property or wrongful death, with the indemnity to include reasonable attorney fees, and all costs and expenses, arising directly or indirectly out of any act or omission, whether or not the act or omission arises from the sole negligence or other liability of its agents, officers, employees, or volunteers relating to this event.
